Applicant Data Form Fall/Winter 2008-2009
Continuing University of Michigan Students
Continuing University of Michigan-Ann Arbor students who fall into one or more of the categories below
for the 2008-2009 academic year must submit this Applicant Data Form (ADF) to the Office of Financial
Aid (OFA) when applying for Fall/Winter 2008-2009 aid:
- Students who will graduate in December 2008
- Students who will enroll for only one term (either Fall 2008 or Winter 2009), including students enrolled in University of Michigan-sponsored co-op programs

Students enrolled in the Nursing program in either Kalamazoo or Traverse City
- Students who are parents with child care expenses
- Students who are on Detached Study
- Students who will participate in UM-sponsored study abroad programs (if you will participate in a non-UM study abroad program, contact OFA immediately)
By submitting this form, you are providing supplemental information to be used for calculating your Fall/Winter
2008-2009 financial aid awards.
Note: DO NOT submit your ADF more than once. Submitting duplicate copies of
this form by different means (fax, mail, in person, online) will cause processing delays! For example,
do not submit this form online and then also fax or mail a paper copy of it that provides identical
information.
