2012-2013 Applicant Data Form for Continuing U-M Students
To complete the online Applicant Data Form (ADF), answer the questions below.

By submitting this form, continuing U-M students are providing supplemental information to be used for calculating Fall/Winter 2012-2013 financial aid awards. If you would rather complete a paper copy of the Applicant Data Form (ADF), you may download and print a copy from the PDF below using Adobe Acrobat Reader. If you are applying for a Health Professions Loan, provide information about all family members in your parents' household for 2012-2013, as reported on the Free Application for Federal Student Aid (FAFSA). If you are a parent with child care expenses, provide information about all family members in your household for 2012-2013, as reported on the FAFSA. If you have more than six family members, please send us an email message at financial.aid@umich.edu (include your name, U-MID, and the information about your family members below).
